All access to student data in the Student Success Link requires informed parent consent. You are responsible for distributing and collecting parent consent forms and following the appropriate process for the district that will be sharing the data.


Seattle Public Schools

  • You are required to use the attached form (download below), which includes both your organization's information and PSESD.
  • Update your organization's name and contact information at the top.
  • When complete, send consent forms in batches to the Community Partnerships department, following the instructions on their website.


All Other Districts

  • Use the attached "generic" form. The form has been translated into 8 languages, available below.
  • Update your organization's name and contact information at the top.


Overview
  • In Seattle, there’s a formal process outlined and you must send in the forms to the district.
  • In Highline, you don’t need to send in the forms - just tell us who you’ve collected forms for.
  • In Renton, you need to send the forms but scanning and emailing is fine.
